What if my child has a life threatening medical condition?

Each September we ask the schools to identify to STEO, any students who have a life threatening medical condition by having the parents/guardians complete one of our Life Threatening Medical Alert forms.

The Life Threatening Medical Alert forms that we have are:

  • Medical Condition: Allergy
  • Medical Condition: Asthma
  • Medical Condition: Diabetes
  • Medical Condition: Epileptic Seizure
  • Other Medical Condition

If your child has a life threatening condition please fill out the appropriate form and return it to the school. Forms can be obtained from your school.

The purpose of these forms is to assist the bus driver to respond to severe life threatening situations, for example, a student on the bus who would require immediate attention within minutes of an attack to prevent death.

These forms are kept confidential by STEO, the bus contractor and the bus driver.
